GSH Name Badges

Name badges are required to be worn with UTS uniforms by students on placement for Physiotherapy, Pharmacy, Orthoptics, Genetic Counselling and Speech Pathology only.

To order your name badge please complete your details below, a payment of $10 will be required to confirm your order. Once the Clinical Placements Team have received your order and payment we will contact you via email to arrange a suitable time for you to collect your badge when you are next on campus.
